Within 90 days of completion of the activity, IMED will request that you provide a summary of the results of your activity evaluation as part of the activity reconciliation. For live activities, IMED will ask that you voluntarily provide average participant ratings for each of the questions included in this sample survey. IMED would appreciate it if you would include these questions in your survey, however, adding them is not a condition of the grant. A PDF version of the survey is available.
